PLEASE FOLLOW THE STEPS BELOW TO REGISTER WITH A PURCHASE ORDER 

STEPS 1-2-3 

1. ADMINISTRATION STAFF: PLEASE COLLECT THE INFORMATION LISTED IN THE FORM HERE FROM YOUR ATTENDEES TO ENSURE WE HAVE THEIR INFORMATION REQUIRED TO ISSUE THEIR CONTINUING EDUCATION CERTIFICATES. 

2. PLEASE REGISTER YOUR ATTENDEES:  YOU WILL HAVE THE OPTION TO ENTER IN A PURCHASE ORDER NUMBER. IF YOU DO NOT HAVE A PURCHASE ORDER# AT THE TIME OF REGISTRATION, PLEASE ENTER “TBD” IN THE PO# FIELD. GROUP DISCOUNT CODE (FOR 5 OR MORE REGISTRANTS IS PRGRP20) GO TO REGISTRATION

3. A COPY OF THE PURCHASE ORDER MUST BE RECEIVED BEFORE CONFERENCE IF PAYING BY CHECK. INVOICING WILL BE DONE AS SOON AS POSSIBLE.  PLEASE FAX TO 682-626-5001 OR MAIL TO FEAT-NT: PO BOX 92123 – SOUTHLAKE, TEXAS 76092 (Need no later than February 3rd)
